In consideration of the benefits to be derived, and in view of the fact that the Boy scouts of America is an educational institution, membership which is voluntary, and having full confidence that every precaution will be taken to ensure the safety of my son(s) on these activities, I hereby agree to his participation and waiver all claims against the leaders of these trips and officers, agents and representatives of the Boy Scouts of America. In the event of illness or accident in the course of such activity, I request that the adult leaders act on my behalf and that measures be instituted without delay as the judgement of medical personnel dictates.

I agree to abide by the Scout Oath, the Scout Law and the Outdoor Code while on all Troop activities. Furthermore, I will fully cooperate with any adult leaders, Troop leaders and fellow Scouts. I understand that if I violate this agreement I will risk losing the privilege of attending Troop activities.

Dear Scout and Parents:

This form is necessary and required for all trips. No boy will be allowed to go on any trip unless this form is turned in. We will do everything in our power to protect your son(s). There will always be adequate adult supervision by the adult Scouters and parents of the unit who will be on these trips.
